General information clause (in accordance with Article 13 of the GDPR)
I have been informed that:
1. The administrator of my personal data is: Stowarzyszenie Douzelage in Chojna, ul. Democracy Ludowej 2/1, 74-500 Chojna.
2. The data protection officer at the Douzelage association in Chojna can be contacted by calling the following number: 603 750 639.
3. Purposes and basis of data processing: Your personal data will be processed for the purpose of:
• consideration of applications submitted to the Douzelage Association in Chojna, based on your consent (basis of Article 6 paragraph 1 letter a of the GDPR);
• implementation of the statutory objectives of the Association (basis in Article 6 paragraph 1 letter c of the GDPR);
• implementation of projects (basis under Article 6 paragraph 1 letter c of the GDPR);
• sending information related to the activities of the association to the indicated contact details (pursuant to Article 6(1)(f) of the GDPR - the legitimate interest of the data controller in ensuring contact with a member of the association)
• possible pursuit or defense of claims (pursuant to Article 6(1)(f) of the GDPR – the legitimate interest of the personal data controller in pursuing and defending claims)
• analytical and reporting purposes, which constitute the implementation of our legitimate interest in this respect (basis in Article 6 paragraph 1 letter F of the GDPR);
• archival (evidence) purposes, which are the implementation of our legitimate interest in securing information in the event of a legal need to prove facts (Article 6, paragraph 1, letter f of the GDPR).
4. I acknowledge that the provision of personal data is a statutory requirement and its provision is mandatory.
5. The processing of the personal data I have provided is based on legal provisions, in particular the provisions of tax law, labor law and social security law, including for the purposes of determining my capacity to work – Article 6 paragraph 1 letter c of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (OJ EU L 119 of 2016, No. 119, p. 1) – hereinafter referred to as the GDPR, Article 9 paragraph 2 letter b of the GDPR and Article 9 paragraph 2 letter h of the GDPR.
6. Data may also be processed in connection with the pursuit/defense of claims (Article 6 paragraph 1 letter f of the GDPR, Article 9 paragraph 2 letter f of the GDPR), as a legitimate interest of the data controller.
7. I have the right to request from the controller access to my personal data, its rectification, erasure, or restriction of processing, the right to object to processing, the right to data portability, and the right to withdraw any consent at any time. Withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal.
8. My personal data will be processed for the period required by law. My personal data may be processed for the limitation period for employment-related claims.
9. My data will not be processed in an automated manner.
10. I have the right to lodge a complaint with the supervisory authority (President of the Personal Data Protection Office).
